Ordering Prescriptions
In line with many other GP practices, we have decided not to accept telephone requests for prescriptions in the interest of safety. This change will be effective from 1st June 2010.
You may order your repeat prescriptions through the website (click on Repeat Prescriptions to the right of this message) or you can bring your repeat slip to the surgery and put it in the box in the reception lobby. Postal requests are also accepted. If you do not have your repeat slip, please provide a written request to include your personal details and items required.

Repeat Dispensing
For those patients on stable medication, we have a repeat dispensing system that allows six or twelve months of prescriptions to be sent to the chemist at one time. For further information, please refer to our prescriptions page and enquire at the desk if you are interested.

New Reception!
We hope you like our new reception area! It has been re-designed after 20 years to make much better use of space and make it possible for some admin staff to work downstairs, allowing the surgery team to work closer together.
The glass screen will enable receptionists to deal with all enquiries in a private and confidential manner. We greatly appreciate patients waiting behind the screen until the receptionist is free.
The self check-in system is now incorporated into the reception area and using this will help to reduce queues and allow receptionists to deal with other enquiries. It is very easy to use – just tap ‘Appointment Arrival’ on the screen to start and follow the instructions on screen. If you have any problems, there is always someone available to help you.

Yelverton Surgery covers a large rural area extending from the north of Plymouth across to Dartmeet and Hexworthy in the east and Grenofen to the west. We have a branch surgery at Princetown which is held Mondays, Wednesdays and Fridays between 8.40am and 9.40am.

PreRegistration If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

Your NHS Number

Everyone registered with the NHS in England and Wales has their own NHS Number. It is the only national unique patient identifier, used to help healthcare staff and service providers match you to your health records. It is an important step towards providing you with safer patient care.

Each NHS number is a unique 10-digit number. The old style of NHS Numbers had a mixture of letters and numbers and was replaced with new style of NHS numbers in 1996.

You do not need to know your NHS Number to receive NHS care, however if you do know your NHS Number it can help the person treating you to find your medical records more quickly and share them more safely with other health care professionals.

The NHS number is becoming more important as systems such as ‘Choose and Book’ use them to identify patients accurately.

At Yelverton Surgery, we already use NHS number on all letters to the hospital and have now started adding them to letters we send out to patients so that you can make a note of your NHS number.

There are a number of ways you can obtain your NHS number from the surgery;

Come into the surgery in person and request your NHS number. We will need to see some I.D to make sure we are giving the NHS Number to the correct person. The receptionist will then write down your NHS Number for you to take away.
Ring in for your NHS Number. We will need to ask you a few questions to verify we are talking to you.
You can also request your NHS number via our website by using the patient request form; we cannot email you your NHS number for security reasons but we can post it to you, ring you at home or arrange for it to be collected from the surgery at your convenience. Please state on the patient request form which option would be best for you.
